VBS returns to Kuttawa First Baptist Church this summer. We are excited to announce our VBS has been scheduled for Sunday, July 11 - Thursday, July 15 from 6:00 to 8:30 PM. We will have Family Night on Sunday, July 18 at 5:00PM.
Knowing that there are risks involved in attending VBS, I will not hold Kuttawa First Baptist Church (KFBC) responsible for accidents/injuries/illnesses that my child may incur during VBS. I will not send my child if he/she has been exposed to someone with COVID-19 within the past 10 days, if he/she is not feeling well, and/or if he/she has a fever or exhibits any flu-like symptoms. If my child develops any flu-like symptoms while attending VBS, KFBC will contact me to immediately pick up my child. Kuttawa First Baptist Church will render first aid as necessary for children during VBS and may call first responders in emergency situations. Every effort will be made to contact a parent or legal guardian in the event of a medical issue. If my child is not riding church transportation, I will personally escort my child to VBS each night and pick him/her up in the auditorium where he/she will be released to me. By my signature, I give permission for my child to attend and participate in VBS and agree with all of the above statements.
VBS 2021 TRANSPORTATION PERMISSION SLIP
I hereby authorize Kuttawa First Baptist Church to transport my child to and or from the church in a church van, bus or other vehicle designated by leadership for this purpose.
FOR INSURANCE AND SAFETY REASONS, I UNDERESTAND THAT NO ONE UNDER THE AGE OF SIX IS ALLOWED TO RIDE TO/FROM VBS.
If for any reason, my child will only be traveling one way (to or from VBS) on each day or on a particular day, I will notify the van/bus/vehicle driver, the VBS Director, or the Minister of Music and Children in person in advance.
I realize it is my responsibility to provide proper restraint devices as required by law for my child or children in my care who are riding the church van or other vehicle designated by leadership for this purpose. (There is no law concerning restraint devices for the church bus. However, KFBC will provide seat belts and children are required to wear them.)
Kuttawa First Baptist Church will not transport a child to or from VBS unless a parent, guardian, or child care provider designated by the parent/guardian is at home when the child is picked up/brought home.
